Chapters Health PACE, a complete healthcare solution that provides a continuum of care to participants at their homes and at the 11,330 square-foot PACE Center, located at 5102 W. Linebaugh Avenue in Hillsborough County.
Chapters Health ia an authorized provider of PACE (Program of All-Inclusive Care for the Elderly) services in Hillsborough, Polk, Highlands and Hardee counties. Chapters Health is enrolling participants in Hillsborough County and is planning to expand PACE services to Polk County in the summer of 2012.
