Chapters Health System has an unyielding commitment to the care and comfort of our neighbors in the last chapter of their lives.

Chapters Health provides hospice care through two well-known organizations — LifePath Hospice in Hillsborough County and Good Shepherd Hospice in Hardee, Highlands and Polk counties. Both are 501(c)(3) not-for-profit, community-based organizations.

Chapters Health hospice care is available for anyone diagnosed to be in the last 12 months of a life-limiting illness. Our services are based on medical need and provided regardless of ability to pay, age, race, creed, sex, lifestyle or illness.

Chapters Health, through its non-profit LifePath Hospice and Good Shepherd Hospice organizations, offers programs such as the Caregiver Program, the Circle of Love and Bethany centers for grieving children, and a Nursing Home/Assisted Living Facility Program. Hundreds of patient/family care and administrative volunteers serve as a vital resource to the hospice program.

There are more than 4,100 hospice programs nationwide; most are Medicare certified. LifePath Hospice and Good Shepherd Hospice are Medicare and Medicaid certified and stringently follow the guidelines. In addition, they are accredited by the Joint Commission, licensed by the State of Florida Agency on Health Care Administration and members of Florida Hospices and Palliative Care and the National Hospices and Palliative Care Organization.
