Give with a Purpose

Every donor who makes a gift to a Chapters Health System affiliate – Good Shepherd Hospice, HPH Hospice or LifePath Hospice makes a difference. Your continued support through a financial donation ensures end-of-life care is available to all individuals, regardless of the ability to pay. Contributions enable us to care for others approaching their journey’s end and provide compassionate support to their families. Circle of Giving features different levels of giving that offer a range of benefits to help donors stay connected to the mission.

What is Circle of Giving?

Circle of Giving recognizes annual donors of $100 or more and entitles donors to benefits based on the cumulative level of annual support. Circle of Giving members provide much needed support for the many programs and services available to patients and families facing life’s most difficult journey.

Join the Circle of Giving and know that you are making a difference at one of the most vulnerable times in people’s lives.

Decade Club

The Decade Club is for donors who have supported the programs and services of Chapters Health affiliates—Good Shepherd Hospice, HPH Hospice and LifePath Hospice—for a minimum of 10 consecutive years. Special recognition is given in the annual report along with an invitation to a  recognition luncheon.

We value your donations and hope that you will remain connected to Chapters Health through the website, volunteer programs or a special event. There are many ways to become involved. Please accept our sincere gratitude for your continued support and friendship.
