The Transitions Program — when you’re not ready for traditional hospice care.
You or your loved one is on an end-of-life journey, but not quite ready for traditional hospice care. Thanks to the Transitions Program — available through our LifePath Hospice organization — you can get the support you need to improve your quality of life, in the comfort of home. And your family can get a much-needed break.
The Transitions Program provides individual and family support with volunteer services, monitors the changing needs of the individual/family, and coordinates community services as appropriate — all at no charge.
Volunteers provide friendship, socialization, light housekeeping, light food preparation, respite breaks and assistance with errands.
To be eligible, individuals must:
- Live in Hillsborough County
- Have been diagnosed with a life-limiting illness
Individuals are eligible regardless of their current medical treatment, whether curative or palliative. No individual shall be refused service due to race, creed, color, sex, age, national origin, religious background, physical disability, sexual orientation, veteran status, financial status or medical treatment.
