How to become a PACE participant.
To be eligible to be a PACE participant, you must be age 55 or older, be certified by the state to be eligible for nursing home care, be able to live safely in the community at the time of enrollment and live in the PACE service area (at this time, Chapters Health is the only PACE provider in Hillsborough County).
Although individuals must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If nursing home care becomes necessary, the PACE program pays for it and continues to coordinate the care.
Enrollment in the PACE program is voluntary. If you meet the eligibility requirements and elect PACE, an Enrollment Agreement is signed. Enrollment continues as long as you desire, regardless of change in health status, until death or disenrollment.
