As a PACE participant, you have an interdisciplinary team of healthcare professionals at your side - whether you’re at home, in the community or at the PACE Center - including:

• PACE primary care physician
• Registered Nurse
• Personal care and center aides
• Physical therapist
• Occupational therapist
• Recreational therapist
• Social worker
• Activities coordinator
• Dietitian
• PACE center manager
• Home care coordinator

As this is your care, you can personally choose additional members of the team, including a family member or loved one, or other professionals such as a personal care worker.

Working with you, the team helps you identify the type of assistance you might need and helps to arrange your long-term care, health and medical services across all settings — from your home to the hospital.

After completing an assessment of your needs, they develop an individualized plan for you. They authorize payment for your services, and they monitor the quality of the services you receive, always with one goal in mind — the most effective and cost-effective way to meet your needs and personal outcomes.
